With access to nine family medicine physicians, three internists, two pediatricians, and three urgent care-devoted mid-level providers, CMM’s primary care team offers extensive medical expertise in both wellness and preventative medicine, and the treatment of injury and illness, for patients of all ages.
Colorado Mountain Medical is one of nearly 2,900 primary care practices nationwide accepted into a new public-private partnership called Primary Care Plus (CPC+). CPC+ represents the nation’s largest ever program to improve primary care. The program provides additional resources and funding to participating physicians with the goal of improving access, delivery and coordination of patient care.
